Our team has reviewed the outage event and has determined that, due to an unexpected condition encountered by our provisioning system, PHP was unable to recover from an error and halted. Manual intervention was required.

Our team rectified the issue, and we have added new checks to our monitoring to prevent this condition from recurring in the future.
